What you want is a Motion Cover Sheet. Some courts have them, some don't. Some courts require a particular form of NOTICE to the opposing party, others only require a certificate of service. And, further, some courts require a Rule to Show Cause (which I'll explain if you email me at klipvm at rcn.com). You need to be very careful about giving legal advice or doing legal forms or documents when you are not an attorney. Some states and some bar associations are very, very sticky about this. If you tell me what state and county you are dealing with, perhaps I can find some forms on line for you. I do this all the time as a legal secretary, so I probably know some tricks of searching for legal stuff.
