I need to write one, and I really don't have any idea other than my name, date and when I intend to vacate. Is there really more than that required?

I feel silly asking, however I haven't written one in years..p.s. I'm moving :)

Dear Landlord:

Pursuant to the terms of my lease, I am required to give you (X) days notice to terminate my lease. Please consider this letter notice that I will vacate my apartment at (address) as of (x date). (If you have a deposit, add: Please make arrangements so that we can jointly inspect the apartment between (x date and x date), to facilitate the return of my deposit. I will return all keys to you when we inspect the apartment.)

If you have any questions, please contact me at (telephone number) between (hours).

Catherin, if it isn't a problem, give your work number, and say Monday through Friday, between 9:00 a.m. and 4:00 p.m. (or whatever your work hours may be).

Catherin, if you have a form of mailing that gets a signed receipt when it is delivered, use that. If you are going to hand the notice letter to the landlord or landlord's agent, make a copy of the letter and ask the person who receives it to write "Received (x date)" and sign it, so that you have proof that the notice was received. Usually a "notice" isn't considered received until it is delivered, so if you are going to mail it, allow enough time for mail delivery so that it gets there a day or two before the minimum amount of notice time required.
